Experienced Adult Family Home AFH Behavioral Support Manager

Kaukauna, WI - Full Time

Agape of Appleton, a non-profit human services agency providing residential support services to individuals with differing abilities, is seeking a highly experienced individual to lead all aspects of our 2-bed Adult Family Home as a Residential Case Manager.  This leadership position is instrumental in providing case management, program management, staff supervision, and direct care to individuals with complex behavioral support needs. 

Key Responsibilities: 

  • Provide leadership and guidance to a direct care staff of approximately 10-12 employees, including training, coaching, mentoring, and performance management 

  • Provide direct care support to individuals based on program needs and staffing levels, including personal cares, medication administration, behavioral intervention, health monitoring, and accompanying to medical appointments 

  • Develop, update, and implement individual support plans, behavioral support plans, strategies, and approaches 

  • Coordinate with the service provision team, including guardians, case managers, nurses, and vocational personnel 

  • Assist with assessments, intakes, and placement of supported individuals 

  • Ensure adherence to all regulatory, financial, and safety requirements 

Qualifications: 

  • A minimum of 3 years of experience in a leadership role as a manager in a direct care setting is required 

  • Previous experience supporting individuals with complex behavioral needs, including experience in autism and trauma-informed care is required 

  • Previous case management experience is preferred 

  • Sufficient experience and/or education to meet the requirements of DHS 83.15, or the ability to acquire and maintain such requirements through Agape-sponsored training 

  • Exceptional leadership, interpersonal, communication, and organizational skills are required 

  • Must be at least 21 years of age 

  • Maintain a valid driver license with excellent driving record 

Comprehensive Pay and Benefits: 

  • Well qualified candidates will earn a regular hourly rate of $26.00/hour, and an overtime rate of $39.00/hour for hours worked over 40 per week
