Support with purpose. Lead with intention. Make a difference.

Agape of Appleton | 8-Bed Coed Residential CBRF Home $22-$26/hr
Are you a natural leader who believes in great care starts with knowing the person behind the support needs? Do you thrive in a role where you can support individuals, empower staff, solve problems, and help create a home where everyone feels safe, valued, and respected?
Agape of Appleton is looking for a Residential Case Manager (RCM) to lead our team in an eight-bed residential home supporting adults with developmental disabilities. This is an opportunity to combine hands-on leadership, relationship building, program coordination, and meaningful direct support.

 What Makes This Role Special?

You won't just manage a program. You'll help create a home.
You'll get to know the individuals who live there, understand what matters to them, advocate for their choices, and help them live lives filled with dignity, connection, and meaningful experiences. You'll also lead the staff team that makes that support possible.
Our home supports individuals with a range of needs, including both ambulatory and non-ambulatory individuals. Some individuals require specialized support with feeding tubes, mobility, and transfers using sit-to-stand equipment or Hoyer lifts. We're looking for a leader who takes safety seriously, embraces person-centered care, and helps staff feel confident and prepared to provide quality support.

What You'll Do

  • Lead and inspire your team. We are looking for an individual who can provide staff supervision, coaching, training, feedback, and accountability to build a strong, dependable team.
  • Champion person-centered care. Ensure each individual is treated with dignity and respect while supporting their choices, goals, independence, and community involvement.
  • Coordinate individualized support. Help develop, implement, and monitor individual support plans in collaboration with families, guardians, and care teams.
  • Keep the program running smoothly. Oversee staffing schedules, documentation, appointments, required reports, and day-to-day program operations.
  • Promote safe, quality care. Support staff competency in specialized care routines, mobility and transfer procedures, feeding tube protocols, medication management, and emergency response, as appropriate to each individual's plan.
  • Be a hands-on leader. Work alongside staff and individuals when needed, help navigate challenges, and model the compassion and professionalism you expect from your team.
  • Build a positive home environment. Help create a culture of teamwork, consistency, communication, and respect. Focusing on making it a home and a caring work environment.

What We're Looking For

  • A compassionate, dependable leader who believes in the potential and value of every individual.
  • Strong communication, organization, problem-solving, and decision-making skills.
  • The ability to coach staff, navigate challenges, and hold a team accountable with fairness and professionalism.
  • Flexibility, initiative, and the ability to manage competing priorities in a dynamic residential setting.
  • A willingness and ability to perform the essential functions of the role, including required direct care and physical support duties.
  • A bachelor's degree in human services or a related field is preferred, or an equivalent combination of relevant experience and training where permitted.
Experience in developmental disabilities, residential services, staff supervision, or complex support needs is a plus.

Why Join Agape?

If you're looking for more than a desk job, this role offers the opportunity to build meaningful relationships, develop a team, advocate for individuals, and see the impact of your leadership firsthand. Every day is different and engaging.
You'll be part of a team working toward something bigger than completing a checklist: helping people live safe, meaningful, and fulfilling lives in their own community.
But wait, there’s more…This doesn’t even include our amazing benefits! We have paid training, a wide variety of health insurance plans to choose from, including $0 premium plans, low-cost dental, vision, short-term disability, life insurance plans, and 3 weeks’ vacation/6 days of PTO in the first year of employment (accrual based).

Voluntary Self-Identification of Disability Form CC-305
OMB Control Number 1250-0005
Expires 07/31/2029
Why are you being asked to complete this form?

We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years.

Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. If you want to learn more about the law or this form, visit the U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) website at www.dol.gov/ofccp.
